The Hospice Team
The hospice team members collaborate together toward the goal of controlling pain and symptoms. This allows the patient and family to take advantage of many special talents as they cope with the complexities of living with a terminal illness. Our team consists of:
Personal Physician
Coordinates the medical care
Medical Director
Provides medical direction for the Hospice team and serves as a resource person to the team
Nurses
R.N.'s and L.P.N’s make home visits and assist with all aspects of the patient's care
Home Health Aides
Make home visits to assist with personal care
Social Workers
Assist the patient and family with legal and financial services, provides counseling, coordinates with community and government resources when available
Chaplain
Assist the patient and family with spiritual needs and grief process for all denominations
Occupational, Speech and Physical Therapists
Assist the patient to achieve the maximum amount of function
Dietitian
Assist with nutritional needs
Pharmacists
Assist with medication management
After-Hours Case Manager
Nurses are available and on call 7 days a week...24 hours a day
Volunteers
Are available to be a "Good Friend or Neighbor" assisting the patient and family by sitting with the patient, running errands, etc.
Bereavement Support
Is provided to the family or significant others for 13 months after the patient's death. Support is provided through support groups, individual counseling, mail and phone correspondence.
